Alternatively, if you would like to donate on a regular basis please print out the attached form (below) and send the original to your bank and either post or fax a copy to Ataxia Ireland – details as above.

All donations, no matter how small, will be greatly appreciated and each will be acknowledged.

Standing order form

Tax Relief under Donations Scheme

If you do not already have one in place, please consider starting a Standing Order for Ataxia Ireland – a mere €250 per annum (€21 per month), with tax refunds under the Donations Scheme, will yield €362 for our charity.

There are two separate forms as follows:-

CHY3 – valid for Direct Debit/Standing Orders of €250 or more. (This is an Enduring Certificate which lasts for up to 5 years)
CHY4 – valid for all donations from 2013 of €250 or more.
